All members who use the exercise room must obtain a Recreational ID card at the Village Office. No other ID will be accepted.

Members of the exercise room must comply with ALL RULES stated herein and posted at the Exercise room. Members should be familiar with these rules. The Village may amend rules, as required, for the safe and proper operation of the facility. Exercise room privileges are restricted to Members as described below:

Members may have use of the Exercise room facility upon presenting a valid Lake Success Recreational ID card for entry into the Exercise room area during the 2006 season. No one will be permitted into the Lake Success fitness area without his or her valid ID card (ID cards obtainable at the Village Office).

Children under 16 are NOT permitted use of the Exercise room -Unless working one-on-one with parent or approved trainer.

APPLICATION PROCEDURE:
Application cards and Waiver & Release Form are enclosed. All members wishing to use the facilities MUST have a Lake Success photo I.D. Members should bring in or send their previous year's ID with their membership cards, applications, signed Waiver & Release Form and locker number and/or membership fee (payable to the Village of Lake Success) to the Village Office for 2006 Exercise room validation sticker. Exercise room cards require four 1" x 1" passport type photos. The Village Office has a digital camera to take photos at the convenience of members (members may provide their own photos of the required size and type. Photos may also be emailed to the Village Office at lsoffice@optonline.net). All member applications must be completed in duplicate, signed by all members of the family 18 years of age, and older. The Waiver & Release Form must be signed by all members of the family 18 years of age, and older. Parents will be required to sign the Waiver & Release Form "As Guardian For" any minor under the age of 18. All members will be required to possess ID cards. ID cards MUST be shown to exercise room attendant upon request. No one will be permitted into the facility without valid fitness ID cards. The attendant will refuse entry to any person who does not have a Recreation ID card. Any attempt to tamper with the Recreation ID card issued by the Village will result in the ID card being withheld pending action by the House Committee.

Tuesday - 6:00 a.m. to 9:00 p.m. Friday - 6:00 a.m. to 7:00 p.m.
Wednesday - 6:00 a.m. to 9:00 p.m. Saturday - 7:00 a.m. to 6:00 p.m.
Thursday - 6:00 a.m. to 9:00 p.m. Sunday - 7:00 a.m. to 6:00 p.m.

LOCKER FACILITIES: Lockers may be rented on an annual basis (if available). Members are entitled to the use of a locker while in the Exercise room only, when available at no charge. Please see a locker room attendant for a designated locker. The Village and its employees are not responsible for lost or stolen items.

ONE-ON-ONE FITNESS TRAINING: Arrangements must be made with the Exercise room staff for one-on-one fitness training. Outside trainers must register with the Village Office.

EXERCISE ROOM RULES: The fitness center attendant is in charge of the fitness center areas. They are authorized to deny use of the facilities for a period of one day to anyone violating rules. They will report any violations that warrant further action to the House Committee. The Committee is empowered to enforce these rules by suspension of privileges or other appropriate means.

The House Committee with the approval of the Board of Trustees has adopted the following rules. Failure to comply may result in a hearing before the Grievance Committee of the House Committee.

* Members acknowledge that you are physically able to engage in any activity, program, or training provided and agree that all exercises and use of this facility are undertaken at your sole risk. You also agree to accept full responsibility for all personal belongings. Derogatory remarks involving any other member or exercise room personnel will not be permitted.
* All members of a household must sign the Waiver & Release Form, 18 years of age and older, before they are allowed to utilize the Exercise room. Parents will be required to sign the Waiver & Release Form "As Guardian For" any minor under the age of 18.
* Children under 16 are NOT permitted use of the Exercise room - Unless working one-on-one with a trainer or parent.
* Members are not allowed to bring guests to the Exercise room.
* Please be courteous at all times.
* Appropriate athletic shoes only. No street shoes, boots, or sandals are allowed in workout areas.
* Shorts, sweat pants, T-shirts, tank tops, & spandex accessories deemed appropriate by management.
* No belt buckles, blue jeans, or loose jewelry items can be worn in workout areas.
* Allow others to work in during your rest periods.
* Keep hands and feet away from all moving parts and weight stacks.
* DO NOT attempt to repair or adjust any equipment that has malfunctioned.
* Workout towels are provided by the Village and should be used to wipe off equipment and benches after use. Workout towels may not be removed from the exercise room and should be deposited in the bin provided after use. Large towels are for use in the Locker Room ONLY and may not be used in the exercise room or removed from the locker room. Large towels should be deposited in the bins provided in the locker room after use.
* If you are unfamiliar with the use of any equipment, please ask a staff member for assistance.
* Rack all weights after each use.
* If dumbbells appear loose or cracked, report the item to the staff immediately.
* Always use a spotter when attempting maximum weight
* Water bottles with spill proof lids are allowed in all areas of the center.
 

WAIVER & RELEASE FORM

You are using this facility at your own risk, assume all risks, and hold the Inc. Village of Lake Success harmless for your activities.  This facility is not manned 100% of the time.  It is assumed that you have obtained any necessary medical releases needed to use the equipment and/or start an exercise routine.  It is also assumed that you are familiar with the use of all the equipment.

You (each member and all participating family members) agree that if you engage in any physical exercise or activity, or use any Village amenity on the premises or off premises including any sponsored Village event, you do so entirely at your own risk. Any recommendation for changes in diet including the use of food supplements, weight reduction and/or body building enhancement products are entirely your responsibility and you should consult a physician prior to undergoing any dietary or food supplement changes.  You agree that you are voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury, illness, or death. The Village is not responsible for any loss of your personal property.

This waiver and release of liability includes, without limitation, all injuries which may occur as a result of; (a) your use of all amenities and equipment in the facility and your participation in any activity, class, program, personal training or instruction, (b) the sudden and unforeseen malfunctioning of any equipment (c) our instruction, training, supervision, or dietary recommendations.

You acknowledge that you have carefully read this “Waiver and Release” and fully understand that it is a release of liability.  You expressly agree to release and discharge the Village, and all affiliates, employees, agents, representatives, successors, or assigns, from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against the Village for personal injury or property damage.

To the extent that the statute or case law does not prohibit releases for negligence, this release is also for negligence on the part of the Village, its’ agents, and employees. 

If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from.
